How to Apply for WIC in Arizona

Arizona has an online WIC application process — start at myazwic.com, the state's Participant Portal, or visit azdhs.gov/prevention/azwic for a video guide and clinic locator. After you complete the pre-application and upload your documents through the portal, your local WIC clinic will contact you to schedule an appointment. You can also call the Arizona WIC Program at 1-800-252-5942, Monday through Friday, 8 a.m. to 5 p.m., to find a clinic or get help applying.

Before Your Appointment

• ID for every person enrolling — a photo ID for adults; a crib card, birth certificate, or immunization record for infants and children
• Proof of Arizona address — a utility bill, bank statement, or lease with your name and address. P.O. boxes are not accepted
• Proof of income or program participation — if your household receives AHCCCS, SNAP, or TANF, bring a recent award letter showing current eligibility. Otherwise, bring the most recent month of pay stubs for everyone working in the household

You can upload these documents directly through the myazwic.com Participant Portal before your appointment, or bring them with you to the clinic.

What Happens at Your Appointment

A WIC staff member reviews your documents and eligibility, asks questions about your health and diet to assess nutritional risk, and confirms your income eligibility. If approved, your eWIC card is loaded with your first month of benefits and staff walk you through the approved food list and how to shop. Any nutrition or breastfeeding questions you have will also be addressed.

WIC in a Click

Once you're enrolled, Arizona's WIC in a Click system lets you complete many nutrition-education appointment requirements online, on your own schedule, instead of driving to a clinic every time. After finishing a module or podcast episode and a short questionnaire, your benefits are issued to your eWIC card within two to three business days. WIC in a Click is only available to participants referred to it by their WIC office — new applicants should apply through myazwic.com or their local clinic first.

Recertification in Arizona

Your certification period depends on your participant category — pregnant, postpartum, breastfeeding, infant, or child. Your local WIC clinic sets your certification period at your first appointment and will notify you before recertification is due. Benefits are loaded monthly onto your eWIC card and must be used within the benefit period.

Moving To or From Arizona

If you're moving to Arizona from another state, contact your nearest Arizona WIC clinic and bring the Verification of Certification (VOC) form from your previous state. If you're leaving Arizona, ask your WIC clinic for a VOC form before you go so your new state can continue your benefits without starting over. Your Arizona eWIC card only works at authorized vendors within Arizona.
