WIC Clinic Locations

In your state there are multiple WIC clinic locations. At these office locations, if you already receive WIC benefits, you can get assistance for any questions or issues you may have with your benefits or obtain your WIC vouchers/checks for the month. If you are planning to apply for the WIC program in your state, you would then need to call and schedule an appointment.

At your appointment, the WIC staff will go over the program with you and review the documentation you are required to bring to help determine eligibility for WIC benefits in your state. If you are planning to move to a new location either in state or out of state, then you will need to contact your local WIC office location and make the request. In most cases the staff will give you a Verification of Certification Card (VOC), which you will give to the WIC clinic at your new address. This card will provide proof that you are on WIC.
