How to Apply for WIC in Utah

Utah WIC doesn't have a single statewide online application — call or text the WIC clinic at the local health department that serves your county to schedule your certification appointment. Use the clinic locator at wic.utah.gov to find the phone number for your area. Many Utah WIC clinics accept both calls and text messages to schedule.

Before Your Appointment

• Proof of identity for each person applying — a driver's license, birth certificate, Medicaid card, or photo ID. Infants under one year can use a hospital crib card or discharge papers.
• Proof of Utah address — a current utility bill, or a rent or mortgage receipt showing your street address (no P.O. boxes), confirming you live in your local health department's service area.
• Proof of income — pay statements from the last 30 days for each adult in the household (4 statements if paid weekly, 2 if paid every other week or twice monthly). If your household receives Medicaid, SNAP, or the Family Employment Program, bring that documentation instead — electronic verification on a smartphone is accepted.
• Infants and children being certified must be present, along with a parent or guardian.

What Happens at Your Appointment

Plan for roughly 45 minutes to an hour for your first appointment. A WIC health professional verifies your documents, measures height and weight, may take a small blood sample to check iron levels, and talks with you about your family's eating habits and any nutrition concerns. If you're approved, you'll receive your Utah WIC card and first set of food benefits before you leave, along with a short orientation video on how to use the card.

Recertification

After your first appointment, you'll return about every three months for nutrition education and your next set of food benefits — many of these follow-up visits can be done by phone or video call. Pregnant participants are certified through delivery, breastfeeding participants for up to a year postpartum, non-breastfeeding postpartum participants for six months, and children about every six months until their 5th birthday. Contact your local clinic before your certification period ends, since unused benefits don't roll over to the next month.

Moving To or From Utah

If you're moving to Utah from another state, contact the WIC clinic for the county you're moving to and bring the Verification of Certification (VOC) form from your previous state. If you're leaving Utah, ask your clinic for a VOC form before you go so your new state can continue your benefits without a full recertification. Your Utah WIC card only works at authorized retailers within Utah.
