How to Apply for WIC in Indiana

Indiana does not have a statewide online WIC application. To apply, contact the WIC clinic that serves your county directly to schedule an in-person certification appointment. Indiana WIC is delivered through 34 nonprofit local agencies operating clinics in all 92 counties, so the fastest way to get started is to find the clinic nearest you.

Step 1: Find Your Local Clinic

• Visit in.gov/health/wic and use the county clinic locator
• Download the free INWIC app (search "IN WIC" on the App Store or Google Play) and use the clinic finder
• Call the Indiana WIC information line at 1-800-522-0874
• Email inwic@health.in.gov with your county or zip code

Before Your Appointment

• Proof of identity for the parent, guardian, or caregiver, and for each infant or child applying — a driver's license, state ID, birth certificate, or other government-issued document
• Proof of Indiana address — a utility bill, lease, or official mail showing your current address
• Proof of income — a recent pay stub, employer letter, or documentation of Medicaid, SNAP, or TANF enrollment, which alone satisfies the income requirement
• Immunization records for infants and children up to age 2 applying for WIC
• For pregnant applicants: documentation from a healthcare provider confirming pregnancy

WIC staff will ask you to bring the infant or child applying to the appointment in person, since height and weight measurements are part of the nutrition assessment.

What Happens at Your Appointment

A WIC health professional reviews your documents and completes a brief nutrition and health assessment for each applicant to confirm nutritional risk, the federal requirement for enrollment. If you're approved, your eWIC card is loaded with your first month of benefits, often the same day. Staff will also make referrals to Medicaid or other social services if you may qualify.

Recertification in Indiana

Certification periods follow federal guidelines: pregnant participants are certified through pregnancy and six months postpartum (twelve months if breastfeeding), infants are recertified around six months and again near their first birthday, and children are certified every six to twelve months until age five. The INWIC app shows your certification end date and sends alerts before it expires — contact your clinic ahead of time to schedule your renewal.

Moving To or From Indiana

If you're moving to Indiana from another state, contact a local WIC clinic and bring the Verification of Certification (VOC) form from your previous state. If you're leaving Indiana, ask your WIC clinic for a VOC form before you go so your new state can continue your benefits without starting over. Your Indiana eWIC card only works at authorized vendors within Indiana.
