How to Apply for WIC in Oregon

Oregon WIC doesn't have a single statewide online application — WIC is delivered locally, so the process starts by connecting with the WIC clinic that serves your county. Find your local clinic through the Find a WIC Clinic or Store tool on the OHA website, or call 211 toll-free (TTY: 711 or 1-800-735-2900) to be connected. You can also submit the statewide WIC Interest Form and a local clinic will reach out — though this is not itself an application. Some counties, including Multnomah, Washington, Clackamas, Lane, and Marion, run their own online interest or referral forms, so it's worth checking your county's WIC page too.

Before Your Appointment

• ID for every family member applying — driver's license, passport, birth certificate, or other government-issued ID
• Proof of Oregon address — utility bill, lease, or recent mail with your name and address
• Proof of income — recent pay stubs, employer letter, or tax return. If your household is fully eligible for the Oregon Health Plan, SNAP, TANF, or FDPIR, bring that enrollment documentation instead
• For infants and children — birth certificate or hospital record
• For pregnant applicants — documentation of pregnancy from a healthcare provider, if available

What Happens at Your Appointment

A WIC-certified professional reviews your documents, completes a nutrition assessment including height, weight, and a hemoglobin check, and reviews your diet and health history. Nutrition education is described by OHA as "the cornerstone of the WIC Program," so expect a real conversation about feeding your family, not just paperwork. If you're approved, your eWIC card is loaded with your first month of benefits, often the same day.

Recertification in Oregon

Certification periods follow federal guidelines: pregnant participants are certified through pregnancy and six months postpartum (up to a year if breastfeeding), infants are recertified around six months and again near their first birthday, and children are recertified every six months until age 5. Your local clinic will notify you before your certification period ends — benefits stop on the end date, so schedule your recertification appointment ahead of time.

Moving To or From Oregon

If you're moving to Oregon from another state, contact a local Oregon WIC clinic and bring the Verification of Certification (VOC) form from your previous state. If you're leaving Oregon, ask your WIC clinic for a VOC form before you go so your new state can continue your benefits without starting over. Your Oregon eWIC card only works at authorized vendors within Oregon.
