How to Apply for WIC in Alabama

Alabama does not have a statewide online WIC application — enrollment starts with your county health department. Find the health department nearest you through the ADPH county locations directory, or call the statewide WIC line at 1-888-942-4673 (1-888-WIC-HOPE) to be connected to the agency closest to you. Mobile County and Jefferson County participants should contact their local WIC clinic directly, since those counties run their own scheduling process separate from the statewide line.

Before Your Appointment

• The person applying for WIC — you, your infant, or your child — generally needs to be present
• Proof of identity for the parent or caretaker AND the person applying — a driver's license, birth certificate, or hospital birth card for infants
• Proof of Alabama residence — a document with your street address, such as a lease or utility bill
• Proof of enrollment in Medicaid, SNAP, or Family Assistance — or, if not enrolled in one of those, proof of income for everyone who worked in the household during the past month

What Happens at Your Appointment

A WIC health professional reviews your documents and evaluates you or your child for nutritional risk — a federal requirement that includes height, weight, and sometimes a hemoglobin check. If approved, your eWIC card is issued with your first benefit period loaded, and the parent or guardian sets a PIN and learns how to use the card. Enrolling a Newborn If You Were Already on WIC During Pregnancy

If you were enrolled in Alabama WIC while pregnant, you don't need to bring your newborn into the clinic for initial enrollment. ADPH provides a downloadable newborn enrollment form on its WIC apply page — complete it and email it, along with the required proof documents, to your local clinic. If you weren't on WIC during pregnancy, standard in-person enrollment applies. Mobile County and Jefferson County participants should call their local clinic for specific newborn enrollment instructions.

Recertification in Alabama

Certification periods vary by category: pregnant participants are certified through pregnancy and six months postpartum (twelve months if breastfeeding), infants are recertified around six months and again near their first birthday, and children are recertified every six to twelve months until age five. Contact your county health department WIC clinic before your certification period ends to avoid a gap in benefits.

Moving To or From Alabama

If you're moving to Alabama from another state, contact your local county health department WIC clinic and bring the Verification of Certification (VOC) form from your previous state.
