How to Apply for WIC in Oklahoma

Oklahoma WIC has an online enrollment request tool that lets you start the application process without an initial phone call — submit your household information, upload proof documents, and choose your preferred clinic at Oklahoma WIC's online enrollment form. Step 1: Submit Your Enrollment Request or Call

• Online: The enrollment tool asks about your household, lets you upload photos of proof documents, and lets you select a clinic. That clinic will follow up, typically within a few days, to schedule your certification appointment.
• Call: Reach Oklahoma WIC toll-free at (888) 655-2942, or contact a county health department, the Tulsa or Oklahoma County-City health department, or a tribal WIC program directly if one serves your area. Any eligible Oklahoma resident can apply at an OSDH-operated clinic regardless of tribal enrollment.

Step 2: Gather Your Documents

• Proof of identity for yourself and each person applying — a driver's license, passport, birth certificate, or other government-issued ID; hospital birth records for infants
• Proof of Oklahoma residency — a utility bill, lease, official mail, or other document showing your current address
• Proof of income — pay stubs or income documentation for the prior 30 days. If you receive SoonerCare, SNAP, or TANF, bring that enrollment documentation instead — it replaces income proof entirely
• For pregnant applicants: documentation from a healthcare provider, if available

Step 3: Your Certification Appointment

A WIC professional reviews your documents, completes a nutrition assessment, assigns your food package, and issues your eWIC card with your first month of benefits. Oklahoma WIC dietitians can also meet with participants who have special nutritional needs by telehealth, and clinics can often complete parts of the certification process by phone once your online enrollment request and documents are in — ask your selected clinic what's available. Oklahoma also allows a designated proxy to attend certain appointments and pick up benefits on a participant's behalf.

Recertification

Pregnant participants are certified through delivery, breastfeeding participants for up to a year postpartum, non-breastfeeding postpartum participants for six months, and children every six to twelve months until their fifth birthday. Moving To or From Oklahoma

If you're moving to Oklahoma from another state, contact a local Oklahoma WIC clinic and bring the Verification of Certification (VOC) form from your previous state's WIC program. If you're leaving Oklahoma, ask your clinic for a VOC form before you go so your new state can continue your benefits without starting over.
