How to Apply for WIC in Louisiana

Louisiana WIC offers an online referral form, phone scheduling, and in-person appointments. Submit an interest form at mywic.us/participantreferral?program=LA and a local clinic will contact you to schedule your certification appointment, or call the statewide BABY line at 1-800-251-BABY (2229) to find the clinic nearest you. All individuals applying for WIC — including infants and children — must be present at the certification appointment.

Documents to Bring

• Proof of identity for yourself and each person applying — a driver's license, state ID, passport, birth certificate, or hospital birth record for infants; a Louisiana Medicaid card is also accepted
• Proof of Louisiana residency — a utility bill, current lease, processed mail, Medicaid card, voter registration card, or recent pay stub showing your address
• Proof of income — pay stubs or other income documentation for the prior 30 days. If your household receives Medicaid, SNAP, or TANF, bring that enrollment documentation instead — it replaces income proof entirely
• For infants: one clean, dry diaper for weighing at the appointment
• For foster children: foster care or adoption papers

What Happens at Your Appointment

A WIC health professional verifies your documents, conducts a health and nutrition assessment including height, weight, and hemoglobin measurements, determines your household's food package, and issues your eWIC card. Louisiana WIC also offers virtual nutrition education through wichealth.org, accessible in the myWIC app, in addition to in-person classes. Benefits are added to your card during WIC appointments rather than loading automatically each month, so keeping your appointments matters.

Recertification

Pregnant participants are certified through pregnancy and six months postpartum (twelve months if breastfeeding). Infants are recertified around six months and again near their first birthday. Children are certified every six to twelve months until their fifth birthday. The myWIC app shows your certification status — contact your clinic before your period ends to avoid a gap in benefits.

Moving To or From Louisiana

If you're moving to Louisiana from another state, contact a local WIC clinic and bring the Verification of Certification (VOC) form from your previous state. If you're leaving Louisiana, ask your clinic for a VOC form before you go.
